Self-Pay or Out-of-Network
▪ Initial Diagnostic Evaluation (60 min) – $180; Subsequent sessions (45 min) – $150
▪ If you have an insurance for which I am not in network, I will provide an invoice or superbill that you can submit directly to your insurance company for reimbursement. Most insurance companies reimburse 60-100% of session fees depending on the insurance plan. I encourage you to contact your insurance company prior to your visit to discuss the reimbursement process for out-of-network services.
Payment are due in full at the time of the service. Case, check, credit cards (Amex, Visa, Master Card, Discover, or Diners Club), and FSA or HSA cards are accepted.
Cancelations or rescheduling must be arranged at least 24 hours in advance. If a session is missed without prior notice or canceled/rescheduled within 24 hours of the session, you will be charged for one half of a fee for a scheduled service will be charged, unless it is determined that an unavoidable emergency was involved.
